Louisiana RHTP Status Update

Louisiana is using its Rural Health Transformation Program (RHTP) funding to tackle one of the state’s biggest healthcare challenges: how to deliver sustainable, connected care across large rural regions facing workforce shortages, high chronic disease rates, and limited access to specialty services.

With more than $208 million in first-year federal funding, Louisiana leaders are building a long-term strategy focused on strengthening rural healthcare infrastructure, expanding clinical access, and improving coordination between providers across rural parishes. The state could receive more than $1 billion over the five-year life of the program.

Rather than focusing on a single transformation initiative, Louisiana’s approach centers on building statewide systems designed to support rural providers over the long term. Current priorities include expanding telehealth and tele-EMS programs, strengthening maternal and behavioral health services, improving emergency response coordination, modernizing technology infrastructure, and supporting workforce recruitment and retention efforts.

The state is also investing heavily in interoperability, data infrastructure, and digital health capabilities to help rural providers better coordinate care and improve operational efficiency. Louisiana’s strategy includes support for community paramedicine programs, clinical training partnerships, and value-based care models aimed at improving sustainability for rural hospitals and clinics.

To support implementation, Governor Jeff Landry recently established the Office of Rural Health Transformation and Sustainability along with a statewide advisory council focused on oversight, funding coordination, and long-term planning.

As rollout efforts continue, Louisiana is positioning RHTP as more than a funding opportunity. State leaders are using the initiative to build a more connected, resilient rural healthcare system capable of delivering higher-quality care closer to home for communities across the state.
