Rural Health Transformation Program (RHTP)

Understand what’s changing — and how rural hospitals can engage

What Is the Rural Health Transformation Program (RHTP)?

The Rural Health Transformation Program (RHTP) is a $50 billion, five-year federal funding initiative (2026–2030) designed to strengthen rural healthcare systems, including Critical Access Hospitals (CAHs).

While all states have received initial RHTP funding, how those dollars are used will depend on execution, measurable progress, and hospital readiness. First-year (2026) awards average approximately $200 million per state, within a range of $147 million to $281 million.

Unlike traditional grant programs, RHTP ties funding to demonstrated outcomes, creating an opportunity for CAHs to focus on sustainable improvements in access, operations, and financial stability.

Importantly, it is not too late to engage. Many states and hospitals are still shaping priorities and metrics, and early engagement can help CAHs position themselves for long-term support.

What RHTP Focuses On

RHTP supports state-led efforts related to:

01

Preventive health and long-term outcomes

02

Sustaining access to essential services

03

Technology-enabled care and coordination

04

Rural Care Delivery Models

05

Funding: What’s Different This Time?

RHTP uses a stage-gate funding model, meaning continued funding depends on documented progress.

For rural hospitals, this may mean:

Participation will require careful planning and realistic alignment with operational capacity.

What This Means for Rural Hospital Leaders

Rural hospital leaders already wear many hats. RHTP introduces new expectations without taking any of the existing ones off. At the same time, it creates an opportunity to step back and be more intentional about financial stability and long-term sustainability.

When approached thoughtfully, RHTP can help bring clearer priorities, better visibility into performance, and more stable operations over time.

There is real opportunity here. And with the right planning, the added responsibilities can be managed—without overwhelming the people already carrying the load.
