Minnesota is beginning to operationalize its Rural Health Transformation Program (RHTP) strategy after receiving more than $193 million in first-year federal funding aimed at strengthening rural healthcare across the state.
Led by the Minnesota Department of Health (MDH), the initiative is focused on helping rural providers improve financial sustainability while expanding access to care through technology modernization, workforce support, and community-based care delivery. The state’s proposal was developed through extensive engagement with hospitals, providers, and local stakeholders throughout rural Minnesota. Minnesota’s transformation strategy centers around several key priorities:
-Expanding telehealth and remote patient monitoring capabilities
-Improving chronic disease management and preventative care access
-Supporting workforce recruitment and retention in rural communities
-Strengthening cybersecurity and operational infrastructure
-Encouraging collaboration between rural hospitals and regional providers
-Advancing value-based care and alternative payment models -Investing in data infrastructure, analytics, and AI-supported healthcare tools
State leaders are also emphasizing practical, locally driven solutions rather than large-scale system overhauls. Part of the strategy includes helping communities evaluate and strengthen care delivery models based on local needs, including outpatient care, emergency services, post-acute care, and behavioral health access.
Minnesota is currently preparing funding rollout strategies and implementation frameworks, with additional opportunities for rural healthcare organizations expected throughout 2026. A strong emphasis is being placed on measurable outcomes tied to financial stability, operational improvement, access to care, and long-term sustainability for rural providers.
