Understand what’s changing — and how rural hospitals can engage
The Rural Health Transformation Program (RHTP) is a $50 billion, five-year federal funding initiative (2026–2030) designed to strengthen rural healthcare systems, including Critical Access Hospitals (CAHs).
While all states have received initial RHTP funding, how those dollars are used will depend on execution, measurable progress, and hospital readiness. First-year (2026) awards average approximately $200 million per state, within a range of $147 million to $281 million.
Unlike traditional grant programs, RHTP ties funding to demonstrated outcomes, creating an opportunity for CAHs to focus on sustainable improvements in access, operations, and financial stability.
Importantly, it is not too late to engage. Many states and hospitals are still shaping priorities and metrics, and early engagement can help CAHs position themselves for long-term support.
RHTP supports state-led efforts related to:

03 Technology-enabled care and coordination
04 Rural Care Delivery Models
05 Rural workforce recruitment and retention
RHTP uses a stage-gate funding model, meaning continued funding depends on documented progress.
For rural hospitals, this may mean ▾

Participation will require careful planning and realistic alignment with operational capacity.
